无障碍资源办事处

The 无障碍资源办事处 at Indiana State University collaborates with students and the campus community to provide equal access to education and housing for all students, 不分性别, 性别, 性别认同或表达, 性取向, 比赛, 种族, 宗教身份, 资深地位, 年龄, 国家的起源, 和/或残疾. To request academic or housing accommodations, please complete the following steps:

2) Attach documentation to the intake application (See below for documentation requirements).

3) Accommodation requests will not be reviewed until the intake application and documentation are received. 一旦收到入学申请和文件, the 无障碍资源办事处 staff may connect with students to determine reasonable accommodations. Please note that it can take up to two weeks for a response from office staff. Watch your ISU email for correspondence from AARO staff or notifications via Maxient.

That documentation can be a formal letter on office letterhead from the medical provider that includes the following information

  • IEP计划/ 504

  • 住宿提供者表格: please note that while this form is a guide to ensure accommodations providers give all the types of information, care providers will likely need to attach additional p年龄s to thoroughly address the question due to space constraints.
  • Letter from a healthcare professional on official letterhead containing:
    • 诊断书
    • A Statement of Recommended/ Prescribed  Accommodations that match the accommodations you have asked for on the intake application. 例如,如果你是 requesting a private room, your accommodation provider must include that recommendation.
  • Also required is A Statement of how the recommended / Prescribed accommodations will help to alleviate conditions of the 诊断.  
  • The Letter must also contain legible name and signature of accommodation provider along with address and contact information for verification purposes.

  • 学生 who wish to request accommodations must fill out the intake form and attach supporting documentation.

  • 学生 should reach out to AARO about changes in class schedules so appropriate faculty is informed of approved accommodations.

学生 have the right to appeal decisions made by the 可访问性 and Advocacy Resource Office. An appeal of approved or denied accommodations must be made in writing within five calendar days following the date notice is received by the student. A student may be notified of the decision via meeting with staff members, email, or phone.   • Faculty/Instructors determine reasonableness for their own courses based on the pedagogical needs of their instruction.
  • 学生 may be approved for accommodations that provide alternatives to certain types of projects, 包括小组项目和公开演讲的替代方案. These accommodations cannot change the content or core elements of the course, which is determined by the faculty member based on their course policies. The alternative assignment should be of equal value and effort level expected of the original assignment as determined by the faculty member.
